Interstitial cystitis or chronic
interstitial cystitis is a clinical syndrome that affects the lower urinary
system, causing urinary symptoms and pelvic pain in the absence of other
identifiable causes. It is also called Painful Bladder Syndrome.
Diagnosis
Exclusion criteria are accepted:
urinári daytime frequency less than 5 vezes.Urodinâmico with uninhibited
contractions of the detrusor, bladder capacity greater than 350mL or 150mL with
no urgency serves to rule out the diagnosis. Absence of nocturia and frame
duration less than 9 months. At cystoscopy with hydrodistension (80 cm H2O) is
expected: Petechiae - 10 per quadrant - at least 3 quadrants, hemorrhagic
areas, linear fissures, trabeculations, Hunner's ulcer (10% of patients).
Cystoscopy is also important to
exclude bladder carcinoma.
Treatment
The treatment can be behavioral,
medical or surgical. The escollha the best option will depend on careful
medical evaluation.
Treatment may initially be behavioral:
Food (sodas, chocolate, acidic juices), eliminate coffee / cigarette, exercise,
stress reduction, bladder training, biofeedback.
The oral medication can be done with
sodium pentosan polisufato, tricyclic antidepressants, antihistamines and
anti-convulsants.
The topic intravesical drug-like
Dimetyl Sulfoxide.
The surgery is hydrodistension under
anesthesia.
