Diarrhea
According to the World Health Organization (WHO), diarrhea causes the death of one person every 10 seconds worldwide. Is usually associated with protein-energy malnutrition and underdevelopment problems caused by lack of hygiene and treatment systems for water and sewage.
Poor children are the most affected, including Brazil, presenting in 50 to 60 days of diarrhea per year. The death of children due to diarrhea is higher than deaths from pneumonia and vaccine-preventable diseases. In Brazil, a country endemically parasitized, where sanitary conditions are poor in many regions, the parasitological examination is very important in the diagnosis of diarrhea. It can be classified as source osmotic, secretory, motor or exudative (dysentery). In osmotic diarrhea you must have an element osmotically active within the intestine, which draws water into the intestinal lumen.
In the case of secretory diarrhea, the classic example is related to the cholera, in which there is a stimulus complex adenilatociclase and consequently an increase in losses electrolyte. The motor diarrhea is caused by a change in the coordination of intestinal smooth muscle, while in exudative diarrhea, dysentery which corresponds to the patient can lose blood, mucus and pus. In the latter group, may be included infectious diarrhea, affecting the colon intestinal and invasive such as salmonella and shigueloses.
The ideal antidiarrheal should inhibit intestinal hypersecretion, act quickly, not cause constipation and most importantly, have no effect on the central nervous system. An innovative drug is oracecadotril (thiorphan), a new drug that activated by esterases in the gut, the gut acts by esterases island, acts on basic mechanism of diarrhea, ie prevents hypersecretion of water and electrolytes into the gut.
The active drug receptor delta (d) and non-receptor mi (m), as do the opioids that can cause bacteremia. Thus, it produces one of the most serious drawbacks in treating diarrhea which is a decrease in mortality with favoring bacterial growth which can greatly aggravate diarrhea.
The drug acts on the main mechanism of most cases of diarrhea, secretory mechanism. Shows no effect on gastrointestinal motility disorders, central nervous system, the respiratory system or endocrine system. The substance is a prodrug absorbed orally. It is rapidly converted to the active compound and one hour after administration already has the highest peak activity. The racecadotril (thiorphan) does not cross the blood brain barrier and usual dose is 100 mg three times a day.
Treatment and prevention
Drinking 2 to 3 liters of water per day, or other oral rehydration liquid containing sodium and potassium. People who have heart disease, high blood pressure, diabetes, glaucoma can not drink beverages containing sodium because it will harm them.
People with diarrhea usually do not eat right because they think that will hurt, but be sure to eat therefore remained weak with no nutrient in the body. Eat plenty of banana which is rich in potassium, avoid eating fat, with crushed fruit, salad and fiber.
